OSSU ARC Panel Webinar: Social prescribing - The Value of a Holistic Patient Navigation Support on Equity

Social prescribing, is an approach commonly embedded in primary care, to refer patients to resources that can address patients’ unmet health and social needs, and help them access services and navigate through the system. A recent research study demonstrated the benefits of a holistic patient-centered navigation approach to achieve higher and more equitable patient access to needed resources. This model, and the plan for its scale and spread, will be presented at this webinar.
